What's a good gift for a Graphic Designer's birthday? I want to get something nice but not over $10 to $15? I don't want to get something expensive or fancy because it's a coworker / friend and its not expected.

A sketchbook is nice.
A proper one though, not a shitty notebook.

Hardbound sketchbooks are great.

Hardbound with some thick paper is awesome.
I've got one with bristol that is fucking great.
Fuck, even a nice pen would be a great gift.
I'm a fucking pen whore. Pilot V5's are my favorite, I compare everything to that.
I was so pissed when I bought a fucking stainless steel pen and the actual ballpoint sucked so bad.
Unless the dude doesn't care much about pens, it might be hard to find the perfect pen.
I don't know of any designer (myself included) who doesn't have a continuous need for more micron pens.
About $13.00 for a set like this.

Alternatively, a nice new compass (and a pack of replacement leads), and a couple pads of grid paper.
Either of these posts would make me happy.

Other miscellaneous items I can never have enough of:
Cork-backed stainless steel rulers, squares, and triangles
Tracing paper
Calligraphy ink (if they're into calligraphy)
Flash drives and SD cards
Coffee
Art and design magazines (personal faves are Hi Fructose, Juxtapoz, Very Nearly Almost)
Acryllic paint
Sharpies (or Prismacolor markers, but those get expensive af)
Mockups (Maybe a license for PSDcovers, I think it's like $16)
Earbuds (Skullcandy are pretty dependable, about $20)
Storage/Art supply cases
Gas
I could go on, but any of the above or a combination would be kickass gifts.
Do most graphic designers ever touch paint or ink nowadays? Most design offices I've been to just have computers and maybe a Wacom. Those designers hire out illustrations for their "wet work".

Good ones do.
You should always sketch out and plan your designs before you bring it into the computer.
Granted you can do those things on a wacom, but pen to paper is still the quickest and easiest way to get an idea down. Especially without worrying about getting bogged down about software type shit.
I always tend to fuck around with paint color, brush sizes and all sorts of shit that doesnt matter. Pen and paper is simple and it works.

Ink, paper, and cardboard (for prototypes) are musts for any design studio. I've never seen one that regularly use paint though

I do, but I also sculpt and work in watercolor and charcoal. I came from an art background before I got into design.
