Big non-tech companies often cheap out and cut corners when it comes to their computer systems. They think that since their legacy code is self-contained and has not been hacked that it can be maintained by one or two indians or maybe a team of disinterested contractors (who are all working other jobs on the side.)
Like all big breaches like this, it starts with one dumbass making a mistake.
