it is more technically challenging to program ransomware that is secure and also capable of decrypting, the usage of PKI is required, decentralization via P2P, and the problems when you need some form of trusted middle man without an SSL certificate.
whilst ransomware that doesn't decrypt merely has to generate a cryptographically secure key for AES or whatever and encrypt, and boom, secure and simple.

Hospitals don't get these issues because the ones that hold sensitive data are only connected through intranet, running vanilla Windows XP, and all the other ones (the clerical garbage PC's which can still hold legally sensitive data) generally have a solid network firewall set up from day one, and never change their software.

If, after a few days/weeks, it starts getting reported that they don't provide the key upon payment then what do you think is going to happen?
