I couldn't recommend one if I think they're all comparable. Plus the two you're awarding bonus points for avoiding are the two where I got my ideal username. Honestly I think that's more relevant than the service.

Been using FastMail for a couple years and am fairly satisfied with it. It's not free, but $20 a year isn't going to break the bank for most people (I use the $40/year level myself). Plus, since it's not free, it has fewer users than the popular free services, so you have a better chance of getting a username you want. And you can have aliases, which are like extra usernames.
