If you live in America, then yes, the ISP is required BY LAW to put on your statement every website that has been accessed. This is safety precaution so you know if someone is stealing your internet.
As for question 2:
Incognito/private will not hide it from your Internet Provider's statement at the end of the month, but it does protect your browsing from eavesdropping (i.e. gov't watching what sites you visit).. Only your ISP can view what sites you visit incognito, the gov't needs a warrant, meaning they need to be watching you specifically to view private traffic
