No you don't. You register for an account with e-hentai, which is free, and install the chrome/firefox/whatever plugin or edit your cookie with your login and that's it.
I've been using exhentai for 6 years and never once seen an ad or touched flash or anything. Shit just works, no questions asked, everyone mind your own business kinda deal.
