Its been just completely freezing lately. The entire computer mouse and keyboard will freeze during a game and quit working altogether. Plus a lot of the games I have been playing (mostly MMOs) have been disconnecting me I guess due to lag or something.

Well your CPU util only got to 14%, so that doesn't tell us much. (looks like those 50%'s are a bug. Does that on my Xeon X3450 [basically same processor])
GPU only got to 15% and never even went above idle clocks.

Grab Prime95 and run it.
