I tried low carb 3 years ago and lost but gained back weight. Lost a lot of weight just tracking calories and burn more then I eat.

It's just a way to get you to think about food and what you eat, same as those who go vegetarian/paleo for health reasons.
A balanced "normal" diet is still the best way to go. If they're losing weight then that's fine, but that'll be because they're intaking less calories than they're expending.

You'll be fine, get a neurologist to clear your friend. Lifting increases blood pressure. Stick to low rep focused strength training and your cardio issues won't really matter.

I just started lifting in mid September. I've made some decent progress but from a stranger's perspective I still look like an out of shape noob.

I started running last year, like 6 miles a week at 7min/mile pace. It's helped my cardiohealth and fatburning obviously.
But it's made my calves bigger than any weight workout ever has. I had no idea. I've prob gained about 50% calf size this year.
