臭 is pronounced SHUU (hehe) on its own and it means like an odor or smell. And when you have 臭い (pronounced ku-sa-ii if I remember correctly) it means 'bad smell' or 'bad odor'. However, it isn't literal as it can also mean something like 'unusual scent I like' whether it's good or bad, however it is almost always used (when no verbal context is given, like in this picture) to mean bad smell.

In her hands she's hold a lemon (people slice them and leave the citrus rinds and slices in their shoe to help clear the smell) and a parody of a very popular japanese shoe odor repellent.
