Never.
You just don't get that big, either. Stretching the urethra is hard and takes a lot of effort and pain.
You would need years to get to that point without just ripping your urinary tract to shreds.
Also the lining is terribly sensitive, so putting a hand in would likely hurt like hell.
Stainless steel or medical-grade silicone (or similarly suitable material) sounds are the only things you should put there because anything that has the least bit of an edge will cause pain and bleeding, likely leading to an infection.
I had that once. Had my kidneys infected, the bladder full of gunk and shit, pissed blood, scabs and similar for a week, couldn't move or lie down... Very unpleasant and unnecessary.
In theory the sphincter should be capable of spreading nearly without absolute limit, but it's just not built for that kind of stretching.
That said, it's muh easier to do urethra stuff up to the bladder for women because the way is shorter and no erection gets in the way of putting in a straight object, nor do you have to account for a twitching prostate.

There's been at least one real world example of a woman taking a dick up her urethra without even realizing the guy was using the wrong hole, but she complained of frequent UTIs (presumably from him jizzing into her bladder). Women are more prone to UTI anyway because the design of their opening is more vulnerable, whereas men's remain closed until we piss. Women's are also stretchier than men's because vaginas in general are elastic.
Incontinence from sounding would depend on whether you penetrate the bladder or not. Just widening the urethra would have no effect on the sphincter. But I'm thinking if you did penetrate it every time while using increasingly large objects over the course of years, it would simply grow to the new size and still be able to shut completely.
