I'm not going to say it's literally impossible, but there are a lot of factors that would get in the way of a human hermaphrodite whether fertile or not. The most basic problem is the fact that humans only develop one set of genitalia as a fetus, which starts out ambiguous but then becomes either male or female. In order to have two sets, a mutation that entirely duplicates the sexual system would have to occur. You'd probably need a number of mutations for that to happen, so that already puts the odds at less than one in a million. But there's that case of the reddit guy who has two dicks, right, so it's clearly possible to have two of something. Assuming you've got a fetus with two sets of genitalia, there's another obstacle. The way sex is determined genetically is through the presence of a Y chromosome, which is dominant such that if it's there, the person will be male, and if it's not there, the body defaults to female. Again, it's not impossible for someone to be born somewhere in-between, for whatever reason (I'm not a doctor), but when that happens, they still have only one set of genitalia that are simply somewhere in between male and female. Suffice it to say, genetic intersexuality is not how a hermaphrodite will be born. So, if you want one fully formed set of male genitalia and one set of female genitalia, you're gonna need some mutation to occur such that, on top of the fetus initially developing two sets of ambiguous genitalia, you'd also need the two sets to develop based off of two different chromosomes. The X chromosome alone would be used for the female genitalia, while both the X and Y would be required for the male genitalia (as per usual).

So, to summarize, a human hermaphrodite would require at least one X and one Y chromosome, and the required mutations for the fetus to develop two sets of genitalia AND for those two sets to develop DIFFERENTLY.

My previous post only actually covers the topic of a human hermaphrodite, though. But a futa would look almost entirely like a girl except for the penis (and whatever the fuck else you think a futa should have, but that flame war is not something I care about).

So that's an issue, because the Y chromosome is dominant, so if it's there at all, the person would presumably develop mostly male characteristics during puberty. So you'd need the DNA to develop a mutation such that the Y chromosome is turned "off" for most purposes except to make sure the dick develops correctly. Otherwise, the person's body reads off of the X chromosome to develop all other sexual characteristics.

So yeah. That's most of the problem, from a genetic standpoint. Let's ignore the idea that the doctors and parents of the hermaphrodite would probably have concerns and might decide on corrective surgery.

Also, I totally forgot that some people think futa means shemale, but in the case of shemales it's pretty simple, lol. That's just male genitals on a mostly female body, no female genitals.

(Also, this ignores the fact that in women, the two X chromosomes can rely on each other for proofreading and genetic diversity, so the hypothetical futa would only take her female sex characteristics from her mother's genes. She'd be, in terms of her sexual characteristics, entirely like her mother, except with dick genes from her father. And when you think about it, all males basically have, mutations aside, the same Y chromosome as their father, paternal grandfather, and so on...)

It's totally possible for people to, naturally, not fit neatly in either sex category. It's fairly rare for anyone to get particularly far from either category, especially since it's standard practice to "correct" these deviations. And since they're all random, it's extremely unlikley anyone would look exactly how you want. (Especially if that includes two different sets of genitals, since having two of one kind is already extremely rare)

That said, taking hormones artificially isn't much different from having a strange genetic mutation that changes what hormones you produce, aside from having pretty much no chance of growth defects.
