If I were doing the game, I'd allow both same size and macro/micro scenarios, each with their own advantages/disadvantages (same size requires you to grapple, but it's harder for the prey to hide from you, macro/micro makes it easier to grab but finding the prey is more difficult).

I'm actually fairly confident it's a decently common thing that arouses people. It may not be their #1 fetish and they probably don't even know it exists though.

That and as you said, they might find one image that sets them off and go looking for more and find a ton of vore scenarios that do absolutely nothing for them which might leave them feeling confused, frustrated and guilty.
