Nah, it's nice and smooth. I thought about buying a normal one, but I liked the length and the reviews all made the ribbing sound pretty nice. Either way, it slides in and out nice and smoothly. I think it's mainly the stretch, the one I use is size 12 or 13 (french catheter sizes iirc). You have to be careful with that though, I was too greedy and there is a slight stricture about halfway down my dick. You're right, I think the size numbers in that case are just the diameter in millimeters.
