Like four or five times? I think Spider-Man and Superman was the first, there was a whole spate of them in the 90s. Amalgam comics was the biggest, Busiek and Perez's JLA/Avengers was probably the best.

Currently? Bad blood between upper management, mainly on Quesada's part from what I've heard. Disney and Warner Bros. aren't likely to sign off on one any time soon either.

This. Some great stuff was made but Quesada was the one that drove up the rivalry and allowed Marvel to take shots at DC from time to time. The most infamous was when he told retailers that he would give free Deadpool variant comics for the covers of Blackest Night comics. That meant you'd have to rip the covers off.

Eventually it will happen. In like 10 years the comics will cross over and in 20 the movies will. At some point the movies will use up all the basic stories and go into multiverse stuff and when that becomes old hat all they'll have left is crossing over the big 2. Star Wars/Marvel crossover will probably come first after the sequel trilogy ends.
