If you're talking about 90's Image Comics, it was because quality control took a backseat to an attempt to crank out at least twelve OCs per issue to get the trademarks on them.

Saga: Saga, where sexual issues are featured and there's currently a miscarriage/abortion plot running.

Sex Criminals: Criminals with time-stopping sex, and discussion of Matt Friction & Kelly Sue's various sexual issues.

Walking Dead: Zombies and cool anti-heroes bludgeoning people with barbwired bats.

That's a look at the main Image titles. Whether it's gratuitous or not is up for discussion.

The only the fits "the WOAAH SEX AND GRATUITOUS VIOLENCE AND GORE" is the walking dead, and it makes sake considering the setting and is nowhere as edgy other similar comics like Crossed or even Marvel Zombies.
