It's common here in Maryland, I think it's to skirt around some beaurocratic "you can't let people order burgers medium-rare" shit.
It's also the way they do it here (Colorado)
I went to applebees about 8 years ago and I remember the waitstaff asking questions that seemed strangely worded or unnecessary, as if they were talking to a child. I assumed it was because they have a lot of poor people as customers who don't know how restaurants work.
a lot of darker skinned people don't realize that "well done" has a very specific culinary definition and would get extremely upset when their meat came out dry and tough. so restaurants that have a large dark-skinned clientele have started using "pink or no pink" to avoid confusion and anger.

Applebee's in PA let you choose between medium-well and well-done.
Your selection doesn't change how the burger is cooked.
Applebee's is shit. The girlfriend and I went there about a month or so ago. I hadn't been in years and I remembered why. Everything sucked. Except for the Sierra Nevada Celebration Ale on draft. If I'm gonna go to a chain, it's gonna be Chili's.

The restaurant I work at only does pink or no pink on burgers. It's mainly to streamline the process- on a $15k night, we don't have time to be any more specific.
