jugular? pressure. pressure pressure pressure. again, treat with diesel. you see dark blood and cant control with pressure, then they're pretty much fucked. true arterial bleeding is hard to stabilize in the field. they need a surgeon.

Former emt here with mass casualty/catastrophic/terror response training here. Also double dipped into peds trauma. Without extra skillsets, beginning pay is awful.

Currently work as a medical analyst in Tampa fl. Easily make more, but it was the skillset that I had that got me hired.

botched GS suicides are pretty common. we call them "jerkers". they put the gun under their chin and pull the trigger but jerk away at the last minute. blow the front of their face off and leave their brain relatively unscathed.
usually we just find whats left of the tongue and stuff at tube down to get an airway then transport. it's brutal.

the proper way to shoot yourself is with a shotgun loaded with 00. duct tape it to your back and make sure it lines up with your brain stem (use a mirror). go to the front yard, lay down a tarp for easier clean up. lean your head back and pull the trigger.

It's an abscess. A red bump in the crook of my elbow. It's very warm too, and isn't really sore I would say. The skin is def tender though if that makes sense. Of course I am on heroin so I wouldn't expect to feel much pain anyway.

A- couldn't do anything "invasive". If the thing was serious, we basically held for dear life and kept the guy alive for the professional crews to treat him. Or just floored it to the nearest hospital
B- we were sent mostly to low-tier calls, so a shitload of drunk people, and stuff like that. Saw my fair share of old men drowning in their own fluids and car crashes, though.

you want to get blood flow to it, so yeah, keep heat on it, and keep moving the limb.
if you start to feel shitty, go to the ER, don't risk it.

and as always, prevention is the best cure. make an effort to make your habit clean.

if it gets over 1 cm, then you'll need to drain it. doing this at home is possible, but i REALLY don't recommend it.
if you must, be extra clean. shower with anti-bacterial soap. clean the fuck out of your hands. wear gloves if possible
sterilize everything with alcohol. wear gloves if you can. make sure they're clean.
soak a razor in iso alcohol for an hour. agitate the fluid as you go.
make a super small laceration at the highest point of the abscess. drain slightly.
apply activated charcoal to the laceration, cover with gauze. change out every hour or so, for a day or two.
keep everything clean. if you feel shitty, go to the ER.

but, i live in a big rural county, with an understaffed company. so i've been in charge of some pretty gnar calls.

i am allowed to go beyond my scope of practice when directed by an ER doctor.

i've done a field amputation of someones hand who was pinned in a huge vehicle pile up. there was 6 vehicles involved, and about 15 patients.
my patient was critical with a pinned hand in the wreckage. i called ER, doc said, "alright bud, you ready? break out your pocket knife and TQ, i'm going to walk you through a field amputation."
i'm sure you can imagine the rest.

im fortunate. in a lot of systems, EMT's only drive. i get to do a lot. shitty ghetto company, but hey, it has it's pluses.

i had a lady who had maggots in her ankle/foot. she was a mentally ill hoarder, overweight, semi ambulatory.
the maggots had eaten through most of the top of her foot, and almost all of her ankle. the only thing still intact was the Achilles tendon. i peeled down the sock and almost puked. had to walk out of the house for a minute.
called for fire for the lift assist, to get her out of her shit pile of a house. a few of them puked.
as i was transporting her, just for kicks i asked her "can you wiggle your toes?", and i shit you not, she could. ankle was basically bone and tendon with some shreds of black necrotic tissue, and she could still move her toes. boggled my mind.

15 yo kid was driving his dirtbike on backroads, and decided to cut across the highway. unfortunately for him, probably the only vehicle on that road, a large SUV, t-boned the fuck out of him at 65 mph. he got sucked under the tires and spit out the back. a tire ran over his neck.
we got there and immediately went to work on an airway. his whole face was fractured. i had to move his face around to even find his eyeballs to see if they were PERL. they werent.
medic was getting ready to do a tracho when the doc on the phone with me said, "nah he's wasted. go ahead and call it."

worst part was the girl in her early 20's who hit him. obv not her fault, but as i walked by she was with a LEO, very upset, and she asked me "is...is he going to be ok?"
