>>675585078 high on the femur? get cozy with that crotch and get that TQ on there. if the cut is so high that the artery recedes (as in, a deep cut to the grown), then you need a surgeon. treat with diesel. fuckin drive, advise ER enroute so they're ready for you. jugular? pressure. pressure pressure pressure. again, treat with diesel. you see dark blood and cant control with pressure, then they're pretty much fucked. true arterial bleeding is hard to stabilize in the field. they need a surgeon.
>>675586085 good podcasts. and don't be afraid to pull over and sleep. helps to have a partner who's good at conversation. don't be afraid to sleep on the gurney. soak your gurney in decon for a solid 10 munutes, wipe down like crazy, repeat. i always stock extra linens just in case i need to crash in the back for an hour or two before switching off. but seriously, good podcasts kill the time. or call your gf. or your mom. just pass the time, ya know?
>>675586305 i get paid minimum wage, which is $10/hr around here. after 40 hours, we get time and half. theres a few slight bonuses ($35 for taking a long distance transfer, etc), but ultimately it sucks. i average about 120+ hours of work a week.
i'm not against any industry getting paid what they've lobbied for. until EMS stops being a bunch of whiny pussies and organize, we have no room to complain.
I stuck my fingers into my friends upper thigh wound and clamped his femoral artery by hand for about 20 minutes until paramedics arrived. He survived but I have never known if my actions helped that. Was sticking my hands into his mangled flesh a wise thing to do or did it most likely make a negligible difference?
>>675586316 i work for a fairly rural company, so they're not as common for us. but they do happen. the majority of our calls, and im sure true for most companies, is old ladies who fall down. old people in general.
>>675586343 don't do it for the pay. do it because it will help you grow up. if you're not going to do military, then you should do some form of public service that forces you into places you'd never otherwise see, and serve (or kill) the people there. it's been worth it to me. if you have a strong stomach and like helping people, go for it. do it for a few years. you'll learn a lot.
>>675587058 two hour trip? are you seriously complaining about that? my last long distance transfer was 8 hours each way... that's pretty standard for our company. someone needs to build a fuckin mental health facility closer to us.
>>675587732 i've seen a few nice ones. but usually if they're young and hot, it's a car crash, and if it's bad enough to break out the sheers, then i'm too busy to stare. but yeah, despite death rattles, it's hard to ignore nice tits.
>>675588032 Not so much complaining about it as much as it is just boring as balls. All public aid psyches in the area have to go to Chicago from where were at because they have the only hospital that will take them. Otherwise were pretty lucky I guess, within 3 hours of Chicago, St. Louis and Indianapolis
>>675588636 not really ribs. it's more like making a triangulation of the chest area. so shoulders, and both sides of their abdomen. you CAN do extremities, but theres usually too much artifact (interference like bumpy ambulance, etc)
Shut the fuck up you basic as bicthes, you all think your so fancy with your dcap btls sample opqrst bullshit, all they should have told you was the KED EVERYONE. ALL THE TIME. Use necessary force when applying a cspine collar and everything will be fine
>>675592459 botched GS suicides are pretty common. we call them "jerkers". they put the gun under their chin and pull the trigger but jerk away at the last minute. blow the front of their face off and leave their brain relatively unscathed. usually we just find whats left of the tongue and stuff at tube down to get an airway then transport. it's brutal.
the proper way to shoot yourself is with a shotgun loaded with 00. duct tape it to your back and make sure it lines up with your brain stem (use a mirror). go to the front yard, lay down a tarp for easier clean up. lean your head back and pull the trigger. lights out.
>>675594737 It's an abscess. A red bump in the crook of my elbow. It's very warm too, and isn't really sore I would say. The skin is def tender though if that makes sense. Of course I am on heroin so I wouldn't expect to feel much pain anyway. I'm pretty sure I missed a little bit of the shot, and that's why it's having this reaction. I don't want to go to the hospital if I don't have to.
>>675595414 never had someone hide a "poisoning". but i've had our local native american population abuse the fuck out of our service. we got a call for a girl who purportedly throwing up blood. we got there and she was standing out front with a purse, a blanket, uggs, and texting. she hopped in and enroute i figured out that she wasn't sure whether she was pregnant or not. she wanted a ride from her middle of ass nowhere reservation to the hospital so she could get a pregnancy test. she knew she wouldnt be paying for any of it. she knew if she told 911 that she was vomiting blood we'd go code 3 and get there quicker. those fuckin people man... no sense of community. i'm not generally prejudiced. but i can't stand our local native american population. they suck.
>>675595887 okay... tits appreciated. but can i get a closer picture of the abscess? how big is it? less than a cm? do you have any other medical conditions? allergies? do you have a first aid kit handy?
>>675596510 shes on the res. government pays. it's just shitty of her. our entire county only has 2-3 units at a time. so to take one out of service for a pregnancy test, when she could just take a bus to the dollar store and buy a test, is fuckin selfish. to lie about her symptoms so we go code 3 is reckless. driving lights and sirens is dangerous, for us and for others on the road. what a cunt.
>>675596312 About the size of cm. Maybe a little bigger. It seems to be growing tho. Yes I do have a first aid kit handy. Everything on the internet is telling me to put a hot compress on it. Is that really all I can do, and just hope for the best? Can't take a picture sorry
What are your responsibilities as an EMT over there? I've volunteered for years as an EMT in Europe, and while we were trainer for every emergency, we
A- couldn't do anything "invasive". If the thing was serious, we basically held for dear life and kept the guy alive for the professional crews to treat him. Or just floored it to the nearest hospital B- we were sent mostly to low-tier calls, so a shitload of drunk people, and stuff like that. Saw my fair share of old men drowning in their own fluids and car crashes, though.
>>675596773 you want to get blood flow to it, so yeah, keep heat on it, and keep moving the limb. if you start to feel shitty, go to the ER, don't risk it.
and as always, prevention is the best cure. make an effort to make your habit clean.
if it gets over 1 cm, then you'll need to drain it. doing this at home is possible, but i REALLY don't recommend it. if you must, be extra clean. shower with anti-bacterial soap. clean the fuck out of your hands. wear gloves if possible sterilize everything with alcohol. wear gloves if you can. make sure they're clean. soak a razor in iso alcohol for an hour. agitate the fluid as you go. make a super small laceration at the highest point of the abscess. drain slightly. apply activated charcoal to the laceration, cover with gauze. change out every hour or so, for a day or two. keep everything clean. if you feel shitty, go to the ER.
>>675597652 generally, EMT's do not have a huge scope of practice. usually it is lower tier stuff.
but, i live in a big rural county, with an understaffed company. so i've been in charge of some pretty gnar calls.
i am allowed to go beyond my scope of practice when directed by an ER doctor.
i've done a field amputation of someones hand who was pinned in a huge vehicle pile up. there was 6 vehicles involved, and about 15 patients. my patient was critical with a pinned hand in the wreckage. i called ER, doc said, "alright bud, you ready? break out your pocket knife and TQ, i'm going to walk you through a field amputation." i'm sure you can imagine the rest.
im fortunate. in a lot of systems, EMT's only drive. i get to do a lot. shitty ghetto company, but hey, it has it's pluses.
alrighty... im pretty toasted. my gf has a cold, gave her a fuck ton of drugs, shes been passed out upstairs in my bed. im gonna go pretty soon here and touch her butt then fall asleep. any last minute questions before i leave? i plan to do this again in the future.
>>675599012 i dont trust the government to do it well. but i support a single payer system. watching someone melt down over their impending bill and ramble about how bad their debt is already can be kinda heart wrenching. i'd rather people in debt not have to pay. honestly, all it would take would be an extra percent or two tax on alcohol and tobacco to fund such a system.
>>675599826 it's called "the code 3 club", banging in the ambo or on top of a fire truck while your partner drives with lights and sirens. unfortunately, my company has cameras. ive been trying to figure out a way to bypass though. if i succeed, i will absolutely give a details green text. VERY detailed. ;)
>>675599923 by law, if someone says "LET ME OUT", you stop and let them out. i've let naked people out on highways, i dont care. usually law enforcement immediately shows up and hog ties them and 5150's them. for people who are just already welfare, they just never pay our bills. we defer to a collection agency, and they just dodge them until they die. it happens.
>>675600187 either a stretchy booty, or possibly a prolapsed rectum. happens to old people sometimes. the muscle becomes weak and things just... fall out. we transport, docs go in and stable it back up.
>>675600525 i had a lady who had maggots in her ankle/foot. she was a mentally ill hoarder, overweight, semi ambulatory. the maggots had eaten through most of the top of her foot, and almost all of her ankle. the only thing still intact was the Achilles tendon. i peeled down the sock and almost puked. had to walk out of the house for a minute. called for fire for the lift assist, to get her out of her shit pile of a house. a few of them puked. as i was transporting her, just for kicks i asked her "can you wiggle your toes?", and i shit you not, she could. ankle was basically bone and tendon with some shreds of black necrotic tissue, and she could still move her toes. boggled my mind.
>>675600056 throat slit? havent seen on of those yet. stab wounds? yes. yes yes yes. one of our stations is across from a prison. so yes. lots. it's a gnarly way to die. drowing in blood? absolutely. upper GI bleeds are insane.
while i've never seen a throat slit, i've seen a crushed throat. 15 yo kid was driving his dirtbike on backroads, and decided to cut across the highway. unfortunately for him, probably the only vehicle on that road, a large SUV, t-boned the fuck out of him at 65 mph. he got sucked under the tires and spit out the back. a tire ran over his neck. we got there and immediately went to work on an airway. his whole face was fractured. i had to move his face around to even find his eyeballs to see if they were PERL. they werent. medic was getting ready to do a tracho when the doc on the phone with me said, "nah he's wasted. go ahead and call it."
worst part was the girl in her early 20's who hit him. obv not her fault, but as i walked by she was with a LEO, very upset, and she asked me "is...is he going to be ok?" i said, "no... he's dead. it's not your fault" she burst into tears, and i just quickened my pace and loaded my gear. that call was gnarly.
