I've worked as a waiter when every table was full and there was a line out the door. I know what it's like to be "slammed." And I somehow always managed to find the extra 8s necessary to top off empty beverages. And, remarkably, I never got tipped a penny. One dude gave me $20 (in 2001, so it was more back then) for maintaining good service despite.

I've never not tipped, but I have left shitty tips when it demands it. My friends and I went to an Applebees for dinner one time and the waitress got every single order wrong, ie shrimp for my Jewish friend etc. I try to be calm and understanding about that kind of thing and patiently tell her the mistake, and she was being a bitch about it. When she did finally bring out our orders, my steak was raw in the middle, and I don't mean rare, I mean raw. Like, still cold. I told her, she got really pissy, snatched my plate, and brought it out five minutes later black as charcoal, and didn't say anything.

I've never not tipped, but just for her, I fished the change out of my pocket and left it there as a tip. Like... two nickels and a handful of pennies.

I do however go to a diner where the waitress there is curt but friendly and always goes the extra mile, and I consistently tip her $5 even on a $10 check.
