Water pollution should be reckoned the main cause of fish disease. Water always has germs in it, and it's common sense not to contaminate the water with more pathogens on purpose, but fish can fight off infections very well as long as the water is clean.
If the water is dirty with ammonia, nitrite, or (lots of) nitrate, it compromises basically all the fish's body functions and makes it a sitting duck for viruses and bacteria and fungus.
