the vast majority of the cases where the pull-out method "doesn't work" is because the guy is an idiot and tries to cut it as close as possible, and actually cums a little bit inside before pulling out.

some of the time it's because the guy ejaculated at some point earlier (hours, maybe a day), and hasn't urinated since then, so it didn't get flushed out all the way. and somehow the sperm is still alive. so it comes out with pre-cum.
