What's the etiquette with asking out a girl your friend

You don't if you want to stay friends with your friend. It doesn't matter that he was rejected, he wouldn't be happy with you dating her and you know it, otherwise you wouldn't be asking this question.

Depends on how much of a loser your friend is.

If he's a real friend and a reasonably well-adjusted human being, he'll be happy for you if things work out for you even if they didn't for him. Yeah, he may have some issues with jealousy to deal with but they shouldn't be too severe.

If he's a complete faggot, you pursuing a girl he was interested in and completely failed with will end your friendship. That's not necessarily a bad thing.
