I've been on several dates with her. Sometimes they are great and sometimes she isn't very talkative. I already know she is shy but one week she will try to see me as much as possible and others will put it off. She says she likes me but isn't sure about a relationship.

So Im in my college at the moment and a girl in my class has recently taken an interest in me. she is attractive but is a known manipulator and asshole.
for example she has thrown away perfect, loyal friendships in the past so that she can be with a foreign exchange student. also i first noticed that she took an interest in me when she was drunk in a bar with her boyfriend and jumped up and started hugging me right in front of him (hes a really nice, good guy)

So I've been really unhappy with myself about a lot of stuff, I have no friends, my job sucks, i'm uncomfortable about my sexuality and my secret fetish life. I can't enjoy going out with friends anymore or more hobbies, so I was told that maybe i should consider going to a therapist. So I heard that I have to talk to my doctor first, whats the best way to go about asking to see a therapist? Should I tell them everything thats bothering me as well? Theres some stuff I want to talk about only with a therapist, as I want an unbiased opinion on certain things.

I'm not sure about talking to your doctor, but I think you could just walk to therapist without counseling to a doctor first. Things might be different if you want it to be covered by insurance, but I don't know well.
I have an uncle who just went to therapist like that. It seems that what he needed was to vent out and let out his "secrets" to another person who wouldn't go and spread it to public. If that was all he needed of course, a professional therapist did the job, but what's the difference with just posting in /adv/ or 4chan in general?
You might get "fake and gay" or helpful responses, but at least you get to vent out your feelings and secrets.

So, dating websites.
I have a pretty decent picture, bout a 7/10, with a profile that gives enough detail about who I am, with morals, job, car, plenty of hobbies listed in the interests section.
When I message women, I say hey, say my name, mention something about their profile and how I like a few of their interests, even name dropping things related to them only someone who likes their interests would know about(leaving a question so they have something to reply with(ie a answer)) say I'd love to learn more about them, wish them a good day, and thats it.
I've been using dating websites for 2 years and messaged hundreds of girls in this format, and only one has given me any contact info. A 4/10 fatty who gave me her skype and likes to complain about how much dating sites suck and how no one wants to date her, only pervy guys do, even though I've asked her out a few times but plans always came up.
Oh, and from women who have a kid already, like some bitch who set her profile to say she didnt have kids, when she did have a kid, when I put in my profile not to accept messages from people who do. Didn't even shit the bastard out for a month before looking for a new man.
I've used POF and OKCUPID. I dont want to use tinder cause Im not not looking for a hook up.

I have extreme itchiness on the vulva and noticed some bumps. After scratching a lot it noticed some blood. Alright, so my friend of 20 years is a (now almost formerly) beautiful girl with a ton of intelligence that is throwing her life away. She was prom queen, campus heartthrob, went to an Ivy League school, and was making about $150,000 a year, until everything came crashing down around last October.
She always had alcohol problems, and dabbled in drugs, but she was functional and kept her responsibilities up. Well, she lost her job, and in the same timeframe, her boyfriend (who is 52) dumped her.
She's gone into a hardcore downward spiral. She spends almost every day at the same three dive bars, drinking herself into oblivion with her "friend" (drug dealer), taking pills and getting to the point where the bartender has to get her a ride home. Three of these bartenders have personally contacted me because they're concerned about her wellbeing. Not only does she look sick, but she's letting fat, drunk old men touch her and slip her shit, going home with randoms, forgetting to pay, and all of this shit.
She's not responsive to anyone that isn't totally enabling her - her parents, her ex, me, her old group of friends that are actually her age, no one gets an answer via text or phone for days on end, and her car is always in front of the bar whether she's there or not. I occasionally get to see her, but it's only when I slip into that dive bar and spend time with her and her dealer - and even then, there's an insistence that she doesn't have a problem and she refuses to speak to anyone that shows any concern, literally running away immediately.
Is there ANYTHING I can do here? I drink too much myself, but this is ridiculous. She won't talk to her family at all. Skipped her niece's birth to stay at the bar.

I agree with this... It's just, we can't reliably get her to show up anywhere. It was one of our closest friend's birthdays on Thursday, at a bar, she RSVP'd weeks in advance, and at 6:30 in the evening finally let us know she wasn't coming because she'd been throwing up in the toilet all day.
All I can really think of is a group of us getting together and going from bar to bar until we can finally corner her, but she'd probably just push past us, run away, and never talk to any of us again.
