It's confusing really and depends on how you view it.

Like what if Re:Creator is a fiction in, let's say, Medaka Box's universe.
If Altair pop out, technically she can't modify what is considered real in that universe.
But the opposite can also happen if Medaka Box is a fiction in Re:Creators universe.
So really, you can't define her power ranking since there is too much to consider.

She's a fictional character so unless she literally jumps out into our world, she would get wrecked by Iihiko. He is too meta, his entire schtick is his detachment from the world in which he resides. He can shrug off any damage as though he's us reading the events as they transpire in a manga. He can irreversibly damage people like the manga pages that they're drawn on. He's pretty much a hard counter to anyone who isn't real in our world.
