Right at the end, the movie is about her becoming that way. It was caused by her being unable to truly cope with not being able to see Madoka, and the memory-wiped Madoka telling her that she would never want to leave all of her friends behind. This led Homura to conclude that Madoka wasn't happy being a concept and so she did what she did. Of course, a large part of it was simply not wanting to be without Madoka again.

How could you not know who she is? She's a magical girl who, in a previous timeline, fell into despair, became a witch, and bit Mami's head off. Since Madoka changed the universe she didn't become a witch and just went to Lesbian Heaven.

At the beginning of the movie? Sayaka and Bebe are technically dead, and Madoka was never alive, but get to come back down because Homura's soul needs to be harvested.
