Pretty much, they can be used by a hero but their nature is darker in different ways. They are altered at least.

Saber Alter - Believes that evil should be used to keep order, she was altered by the grail

Jeanne Alter - Fanfiction version of Jeanne created by the grail, as wished upon by Gilles de Rais. This is Gilles ideal version of Jeanne, which hates and curses everything after she was killed. However innately she is not Jeanne D'arc, she is Gilles fanfiction.

Cu Alter - Medb wished upon the holy grail for a version of Cu that lined up with her ideals. She got a heartless version of Cu who only wishes to kill and keep killing, a beast of battle who can only fight to show he's the strongest. It seems that he is the real Cu, but heavily warped. Seems that he's warped by his own dark ideals of what a king is meant to be.

EMIYA Alter - In a certain world-line EMIYA came across an anti-christ like figure who was leading thousands of men into destruction like a moth to flames, he realized that if he left this person alone she would become a beast far beyond humanities control. So he killed his ideals, his memories, and everything he had in order to kill her at all costs. Now he's an emofag coming apart at the seams who can't remember five minutes ago, all he knows is how to kill, ideals are dead.

Saber Alter Lancer - Lancer version of Arthuria who chose to be altered by the grail so that her spear couldn't corrupt her into becoming a divine spirit. Rhongomyriad turns the user less and less human the more they use it, she chose to fight this. However she retains her heart.
