Von indicates where you are from either a region or surname you are representing. For a knight it would read Ritter surname zu lord\country they serve under.

No, in that time frame it's pretty much synonymous.with nobility.
The times where farmers would just say "I'm Bruno from [thatvillage]" are long over.
I am not well-versed in etiquette, so I can't say for sure how you'd address a knight properly, but I don't think adding the Ritter is necessary.

Ritter is the knight equivalent so it would be like having sir before your name to indicate title. Still like you said it may have been different in that time and was most likely only used at formal gatherings.
