Early SS fights and the three named guys you mentioned all dealt with Ichigo trying to tactically work his way around the opponents ability (ex: Ikkaku's ability makes his spear shikai split into 3 sections, Ichigo has to work around this). Kenpachi's weren't abilities, but handicaps, but the same rule applies. If it was just pointlessly waving around powerlevels, then Zaraki would have won, but because of the interesting handicaps he was giving himself, Ichigo was able to overcome him. They waved powerlevel dicks around for the final clash, but even Ichigo winning that has been contradicted by basically everything we've seen of Zaraki since then, oh well.

Welp, Isshin bailing on SS and moving to the real world had the entire family kicked out of the GNH.
The timeline of events is as follows.
Shiba is demoted to branch to an existing GNH following unknown set of events.
Shiba is kicked out of the GNH entirely after Isshin leaves to the world of the living.
Shiba loses even minor noble status after Kaien dies.

It's confirmed that Isshin's brother was the head of the main house and Kuukaku, Ganju and Kaien were from the main house.
