It could easily be the heart that turns into a stone while the rest of the person becomes a fleshy blob not unlike Mitty; surely if it's the curse alone that creates the stone, it's a special case of the curse. They might have used some mechanism of lowering and raising the person and then carving the heart-stone out of the narehate that results.

The weird part is that Prushka became both fodder for someone else's 'blessing' AND a stone. In 41 giraffe told Nanachi about the very powerful desire protecting her, like he could sense it. They are exploring areas with basically no danger whatsoever, the monster that appeared at the very beginning is a VERY rare sight, he should be at the 3rd layer instead. Rare phenomenons can always occur, both with and without someone looking over them, and just because one single child dies they aren't going to make other people searching in deeper areas just go look over the children, especially when they barely bring anything special. You can hardly describe the orphanage as cruel when the alternative is life in the slums. These are children who would otherwise have nowhere else to go, at the orphanage they're trained to make a life for themselves.

In the sort of time period Made In Abyss would be set in, continuing to live as a poor dude, particularly a child, means that you will probably starve to death, or die of disease. The overhead costs for sustaining a single child is considerable, if you aren't already aware. In such a society as MiA, an orphanage that didn't put its kids to work would go out of business very quickly. And then the kids would be back in the slums, dying of starvation and disease. And so we're back to the original problem.
