The only real difference is that anime used to be about pushing the anime. Even if there was a source material you couldn't sell just a prologue.

There's still good fantasy anime, they're just few and far between now. Garo: Divine Flame was fucking great, for example, and that was a spinoff of a sentai/live-action series.

The "alternate world" concept is just a setting, nothing more. There were plenty of isekai fantasy anime in the 80s and 90s that managed to tell a quality story that just happened to take place in an alternate world. The concept itself isn't inherently bad, it's the execution that matters.
