This is the only part in which you are partially right. He did feel guilty but he let that guilt paralyze him. First love or rather, teen love is really complicated, so much more so that people don't awaken to what they know and feel at that point in time until much later. I could use real world examples but that would be bringing 3DPD shit into this anime discussion, so I won't. Gist of it is, it's a love story, but all the baggage that came before dragged their relationship down.

Unfortunately, Ueno's story is one of the things that gets lost in the transition from Manga to Movie.

It's nothing exciting, she is a shallow, but honest girl who falls in love with Shouya and starts bullying Nishimiya to get his attention.

As she becomes an increasing burden to her, it continues, and as Nishimiya fails to give the reaction she expects, getting angry or fighting back, it continues and she starts to believe that Nishimya is manipulating everyone with her "Fake smile act." and using her deafness to her advantage.

Then Shouya becomes a Pariah, which is obviously Nishimiya's fault and she can't hang out with him anymore, (Again, Ueno is shallow.) but ultimately it's the deaf girls fault stealing him away from her.

Then later she finds Shouya again and confesses to him, only to find out that he has changed to some timid introvert and is hanging out with that manipulative deaf bitch who stole him away in the first place. So on and so forth.

Long story short, Ueno is shallow and stubborn, but honest. She is in love with Shouya and earnestly believes that Nishimya is playing an act to steal him from her.
