Well I think Madoka is an exceptionally rare case because she had enough stacked up karmic power to change the system, but the issue was exactly that. Her wish is literally the only one that manages to bypass the karmic system they had established. Even if she wished for more wishes, which we don't know if she had enough power to do so, then she would have immediately turned into a witch before she could use any of them anyways, as we saw before.
