Regarding whether she's the same person or not is intentionally vague, as shown by her inner thoughts in ep 25. The question comes down to whether you consider someone being the same person based on what they know versus their soul - I personally subscribe to the latter.

You could say the same thing about the other characters having a fantastical struggle. Shinji hates his father for forcing him to get in the robot, and can't bond with others ever since he lost his mother to that robot. Misato seeks vengeans against the angels who killed her father. Asuka seeks validation through being an ace pilot. They're all fantastical situations, but deep down they're common human struggles.

Rei's is that of someone who does have an existencial crisis over being raised in an overly restricted way,

Okay, its part of the Ego Salvage plan that took a month to set up, starting at 14:37 of ep 20. Misato says Shinji's name as the operation starts, it cuts to Shinji opening his eyes in what looks to be a hospital, then the people Shinji knows start to say his name with a visual of their name indicating who's saying it on screen. Except for Rei, who looks to be in the same room as Shinji in his 'waking up' just before. Despite this, Shinji doesn't 'come back' just yet and the Ego Salvage is a failure.

As for the whole sequence, I think it's Eva/Yui pushing pleasant memories unto him to keep him there. Which is why the opperation fails until misato starts crying over him and either shinji himself or Yui gives him back.
