Megumi is upset at the fact Tomoya mistreats her. He doesn't see her as a member of his circle and neglects her thoughts and contribution. Not informing her about a significant decision like stalling the project was simply the last straw.

The PV does show additional scenes that aren't there in the LN. We know the AV scene will happen in the following episode, so Tomoya will be hurting and being confused before scene where he sits down and play the game that makes him realize how he was in the wrong. But those scenes are short and hardly narrated/focused in the LN. The anime is filling in space and seemingly developing him relationship with Eriri from a continued point starting from after they have just reconciled.
