Lots of cities were built on little islands, but since that's an optimal position they usually grew quite a bit and expanded beyond the little island, making the river (and the island) just a little part of the city.

You are just looking at the map too late. Île Saint-Louis is actually an incorrect answer because the city was founded on Île de la Cité. But Paris was founded on an island nevertheless.

If you can't cross the river, then you don't want to make a settlement on it. If you can cross the river, then there is no reason why you can't have a city divided by it, or 2 sister cities, one on the river, one right beside it, which would naturally turn into a single city over time.
