I did listen to Nihei confirming on a radio program that he's doing all the work by himself at least as of Sidonia. Not sure for any of his previous works.

As for Hayashida, both she and Nihei do a fashion brand at the same shop (http://m-hz.net/), so it's likely they do have some sort of connection. That said it's also possible that people just linked the two together, which snowballed to the assistant thing.
