Both. The brain and body are always interacting with the environment, they do not exist in some form of vacuum where you can easily measure and diagnose things.
Some people have a predisposition, suicide can run in the family, but trauma and pressure will physically and chemically change the brain.

What he really, provably has is avoidant personality disorder. He also has some bipolar and histrionic traits, most of his outbursts are simply a way of getting attention and collecting pity points in front of an audience.

Asuka demanded to be objectified and demanded that he be the dominant party in the "relationship". Also Shinji is the one that cooks and cleans for two women for the entire show.

He strangled her after putting up with months of abuse from a person that demanded all of his time and attention. while giving nothing in return. Nothing, not even a smile, for months. No one wants to be less than nothing. And it was his projection and not a real person in that scene. He broke down in tears when he tried strangling the real one.

I've tried to keep my anger in check while writing this. I've never really understood how anyone could hate women, but I'm beginning to understand. For better or worse.

I went through a similar dysfunctional relationship in high school. Both of us had hella issues and she was incredibly self centered.

Obviously we were both at fault, we both contributed to the problem. You could argue I didn't care about the girl I was with at the time because I didn't do exactly as she wanted me to do. But one of the lines that resonates with me so well from Eva is when Shinji says to Asuka "how do you expect me to understand you if you won't tell me anything?" Every time I had an issue with my old GF she would yell at me for not just understanding her for no reason.

Though I was not flawless I do think I made an effort to care, but she just expected me to know exactly what to do or say because in her mind that would demonstrate that I "understand her" and apparently that's what a lot of people look for in a relationship, somebody who just "gets them".

It's a common trope that plays out and the stereotype is that women communicate in a way that they expect men to understand without using overt language, but men expect women to be able to communicate what they want explicitly. I don't know if this is what really went on in Asuka/Shinji's relationship but I figured I'd bring it up because that's what I could see myself. This argument does strike a nerve with me too because even if Shinji acted kinda selfish, Misato and Asuka were also trying to use him to fill holes in their own lives.

Forming a relationship doesn't necessarily mean it's a romantic relationship. Working with someone means you have a labor relationship with them, Asuka and Shinji both live, work and study in the same place so their relationship is a bit more than just that, however its not a solid relationship as they each expect the other to understand them without spending the time to talk to each other.

Probably some mess where there is physical attraction but issues growing closer to the other person due to lack of maturity. This is incredibly common in most adolescent relationships and I've seen/still see so many people around me go through this since I was in HS or so.

Honestly I don't think there was a single model of a stable, healthily functioning romantic relationship in Evangelion. A bunch of characters were getting physical or straight up banging, but it always seemed to be for the "wrong reasons", i.e. using people to fill a void or something.

To be honest I felt like what it showed us was that Shinji does better with male friends (Toji and Kenske) because he never turned to those two demanding they mend his childhood issues. Asuka never did that either with the class rep she hung out with (forget her name). For whatever reason the characters in the show turn to people of the opposite sex and seek out validation and emotional fulfillment rather than pure friendship, but when it comes to people of the same sex they often get along just fine. Even Misato felt the need to kiss Shinji at the end for some reason.

Kaiji kept telling Shinji how fucked relationships between men and women can be and this is probably why he said it.

Shinji was the same, except he was extremely passive and fearful of getting hurt, so instead he repressed everything.
The kitchen scene in EOE best describes Shinji's emotional level reaching it's tipping point in a fever dream type scenario. He wanted to 'help' Asuka, but all he did was try to force out her feelings (to him this was what you should do, due to his inability to communicate and try to understand) making her lash out at him, then started to ask her for help when she was angry. Looking down at him in disgust and insulting him for begging for help after he got burned and had and outburst was how he saw her, even though he was being "sincere".
Shinji feels like he's been there from the beginning for her, but it's *her* fault for not understanding him. It's all *everyone's* fault for not understanding him, which is how he sees it. Chocking her was his repressed emotions coming out, all the aggression he felt over Asuka not understanding him.
Asuka is also very bad at communicating, so Shinji's kitchen Asuka is just the outer layer of what she shows.

It's not that they didn't want to be in a relationship (be it friendship or mutual understanding), it's more like they figured the other would understand the other at some point and they'd start from there.

He wants to help, but most of all he expects others to help him. Singling out Asuka as the only one who can help didn't really mean much, but he expected her to help him anyway. When that didn't work he begged for anybody in general, which is when Asuka just silently looks down at him. When he throws another tantrum and lets all emotions loose, he's basically doing what he's done before (running away, quitting, refusing to follow orders or following them incorrectly, inhibiting emotions, etc.) in hopes that someone will reprimand him so he can shift the blame and repress everything again. Getting called pathetic by Asuka only tips him over the edge since he saw it abandonment.

I guess in a way, kitchen Asuka can be seen as the representation of everyone in Shinji's life. His lack of understanding makes him feel like everyone dislikes him in silence. Asuka being the one who has insulted and degraded him the most (out of her own lack of communication and inferiority issues) has him represent everyone's imaginary dislike for him in Asuka's form.
