Kiyoshi constantly lies to Chiyo about everything and places her on a pedestal. They never have any kind of real relationship that isn't based on misunderstandings and Kiyoshi's desire for Chiyo feels more like idolization than real love or lust.

Meanwhile Hana and Kiyoshi know everything awful about each other and Kiyoshi gets pissed on and enjoyed it. The latest chapter has him even getting a boner over it.

The conflict in Prison School isn't whether Kiyoshi will get together with Hana or Chiyo, it's whether Kiyoshi can
a) Realize how horrible he's being to Chiyo and come clean with everything he's done.

During the battle, Kiyoshi's dick gets shown to the whole school. After the battle, Kiyoshi's getting bullied and Hana tells Chiyo to stay away so that she doesn't get bullied too.
Chiyo gives a note to Hana to give to Kiyoshi, which Hana rips to pieces and shoves it in Kiyoshi's mouth. When he spits out the note, it says "Die Kiyoshi-kun" or something. Basically it seems like Chiyo told Kiyoshi to die.
Kiyoshi loses will to live, tries to off himself, and finally Chiyo walks in.

Events happen to affect characters to drive the plot forward, it's not like Kiyoshi and Chiyo are in a bubble. For all we know, the manga could go on long enough for something significant to happen, turn all our expectations on their heads, and end with Kiyoshi piledriving Mari's funparts every night.
