I actually watched Black Lagoon dubbed, and I can say that the first season dub is kinda crap. The second is ok.

English dubs are normally crap because actors can't pronounce Japanese names properly at all. "Come on, Nahrootou, let's show Sah-sookeh who is da real ninjah". I know they are trying their best, but it's just weird.

Yes and no, on supervision nowadays. A couple of dub vas at cons and so on have indeed said that there are often people from Japan who help supervise the dubbing of a series, to a greater or lesser degree depending on the series. For example, when Funimation was dubbing One Piece, they wanted to use somebody, I forget who the guy was, as Luffy, but Toei stepped in, said "no, you're using her" (Colleen Clinkenbeard), and they just had to suck it up.

Back in the late 90s and 00s it might have been less supervised, but there's a lot more control exerted over it now.

I watched an edited version of the Psycho Pass movie that kept all of the Japanese characters subbed with Japanese dialogue, while the foreign American characters used their dub dialogue.
