Shinji was venting.

In fact I kind of had a moment like that now that I think about it. There was this girl that was usually pretty nice, class president even. However she took playfull teasing too far. At this point, he thinks he's gone completely crazy after all the instrumentality visions. In the instrumentality visions, you can see that he wants to see Asuka as a hateful person that just always bashes him about everything and never helps him, hence the first choke.
