they've definitely built up to that with the narration ("that time without thinking, I lost control) and clearly his quirk is capable of breaking people's bones.

it would both make sense and be very satisfying for one or both of bakugou's arms to be totally messed up, say with a bad break through both the radius and ulna at a weird angle.

It's dumb because they live at a school with a nurse whose quirk magically heals all wounds that don't require extensive surgery. And even then it's just surgery enough so that the body heals normally after applying her powers. Unless Deku completely severed Bakugou's arms or something there's no reason for him to have any long-lasting damage of any time.
