From a thematic point, Shinji killing Asuka is a representation of his rage-filled rejection of the shitty, flawed world that he lives in, which outwardly seems to hate him and bring unending harm to him, but that he subconsciously realizes is not evil and actually cares deeply for him.
Asuka being alive post Instrumentality shows that at the last moment Shinji comes to the realization that he does not hate the world and wants to hold onto it in some form in the face of the introspective loneliness which Rei represents as the figurehead of the Instrumentality itself.

In other words, Rei killing everyone represents Shinji's mental barriers keeping everyone out and destroying his connection to the world, while Asuka is the single connection he held onto and is also an Eve like figure to Shinji's Adam, a metaphorical chance to recreate the world (with Rei representing Adam's first wife Lillith if you take the Kabbalah references seriously)

There's also some crackpot pregnancy theories largely planted by Anno to fuck with people. Asuka's period cramps are meant to drive home her feminity in a negative imposing way but shouldn't be read into further than that imo. And herp derp "kimochi warui"
