Tsukihime is a complete fucking joke compared to Fate really. I'm spoilering all of this because they're some of the most important scenes in her routes.
When shiki first came to the tohno house has a kid he was freaked as fuck because his family just got killed on orders from his new dad. He boarded himself up in the shack outside the house. little Comes and tries to get Shiki out. During this scene she says something along the lines of "if you can't trust anyone, you can trust me." Skip foward to the main plot Shiki paranoid and drugged up thanks to kohaku's poison. Once again boards himself up even from Hisui who has been staying with him while he's been sick. She tries desperately to get him to open up and starts crying outside the door and says the same line she said when they were kids. Shiki opens the door after that.

Hisui knew all along Kohaku was drugging Shiki.
She was an accomplice.

You should have noticed this immediatly when the "Nee-san" and "Sister" were used.

The issue isn't whether Shirou knows someone (especially since he has no issues with choking out Shinji like a bitch), it's really more whether the person is a dickish villain that needs killing (Gil, Shinji) or sympathetic villain that needs saving (Sakura, potentially Satsuki). Kirei kinda blurs the lines since he's a dick in Fate but more sympathetic in HF where Shirou kinda likes him but Kirei's doomed anyway.
