With medical advancements many people with crippling genetic disorders can still breed and survive. We can see examples with the male offspring of people unable to reproduce without artificial intervention: the offspring have extremely low sperm counts.

People who are so stupid they would have been grifted for all they're worth and left to die in a cut throat society instead survive to bequeath and inflict their genes on future generations.

Without genetic engineering each successive generation has the potential to become incrementally less fit, that much more stupid, or that much more physically incapable.
