I don't know about it being scary, but Franken Fran had some of the most disturbing and gross shit I've seen in manga to date. A good example is in the chapter where Fran invents a way to make childbirth convenient by placing fertilized eggs inside insect-like pupae that simulate human womb. Horror strikes when household pests like rats and various insects bite through the exterior and lay eggs in them, and 9 months later the bundle of joy opens up for them to find a swarm of bugs spewing out like no tomorrow. That grossed me the fuck out and I never went back to that chapter since. Most of the chapters are tamer in comparison, though. It's a good read if you brace yourself for some cruel and unusual body horrors.
Higurashi was decent, but mostly for the facial expressions.

Scariest manga: probably the manga version of The Ring. I used to own the 1st volume and get seriously spooped if I read it before bedtime.
Ito for me is just a bunch of grotesque images. Very rarely does his work strike any fear in me.
Fuan no Tane so far is the kind of horror that gets me the most. Makes your imagination runs wild, you know. I hesitate to look at any dark corners in my house, or even simply look behind, even though I know nothing is there after reading that.
For people who enjoy the feeling of helplessness, being followed by something evil and you just can't get away, you might want to try reading Ibitsu.
