The scene at the Tsukiyama raid was obviously a hallucination because he was imagining Arima to be where he was (at the rooftop). The hallucination there was caused by how hard he was being pushed and how he was given no choice, which meshed well with his memory of being trained by Arima. The other parts of the hallucination were influenced by how his own former hallucinations and memories disappeared.
