There are tons of antecedents like Rayearth and .hack from before it had become a complete cliche. The current wave seems to be more of an LN trend, so as someone who doesn't follow them very closely because I can't into Japanese and fan translations suck and because LNs suck it's hard for me to guess at what started the recentish trend among novels since I'm mostly exposed to their adaptations.

Translation errors happen in something thats getting done on short notice. In general though the guys at funi or cruncyroll do a job that is as good or better than the better sub groups of old.

Ive been here over 10 years and the last 3 or 4 really have upped the average quality of translations. Anything that crops up now is nothing you didnt see but 10 times worse years ago.

Common ways around it include relying on assumed genre knowledge of the audience, and just not doing an info dump of things the character already knows and conveying those things through environmental storytelling. /a/ of course is well know to react poorly to the latter.
