That rarely happens, and if it did there are situations where you would just let them all die anyways or as many or more than you would sacrifice to save the rest, so you are doing more good in that sacrifice.
Still, it's probably not fair to just arbitrarily decide yourself. You might be wrong in your evaluation of the situation. But if you definitely had to let 40 people die to save 60 by going for one lever over the other to save a train, it would be better to go for the 60. Unless they were nazi's I guess. But then you can torture yourself forever inventing complications.

Because Touma's ideal works out in his universe versus in Emiya's world the ideal is shot down more realistically, resulting in a more jaded man.
His ideal literally only works because the author allows it to.
