Anna was resonating with the Slates via her Strain power, and Dr. Evil wanted to make her into the next Blue King (there wasn't one at that time). Pain seemed to enhance Anna's powers, so he tortured her in numerous ways daily and then healed her back to 100% with his own powers. Slate obviously rejected her becoming Blue given she was destined to be Red, causing more pain. Mikoto et al. saved her because he knew her from back when she was with her aunt, but I don't remember how they found out about it.
