So Decim bring Chiyuki back to her house, or at least a magic holodeck version of her house, where she can see her mom grieving for her.
Chiyuki looses it and pleads Decim to revive her or something.
Decim overwhelmed by his feelings for her cries because he can't deal with the fact that he has to judge her.
The holodeck illusion of Chiyuki's home shatters then Decim and Chiyuki head somewhere (the reincarnation/void elevators maybe)
But what if God is actually Clavis? Clavis wanted an easy time and felt like he would watch from the sidelines for a while. Erased their memories of him and left Occulus in charge.

Why did Harada get voided? Even though he thought of doing something bad during the game, he ultimately redeemed himself a few seconds later. He did nothing wrong.
