It's true that in combat sports, both participants have to go in with the mindset that they'll bypass the usual restraint civilized humans have cultivated all their lives and do harm to another human being. I don't know either where the author is going with that. Here's what I gather:
- Natsuo's talent made him grow a huge inferiority complex. He thought guys have to be strong but his sister is athletically better than him without even trying.
- Natsuo dreams of him being cool and strong (as shown a few chapters ago) and he isn't. The huge disappointment coupled with a mistreatment from the weakling brother, turned into a hatred over years, generally aimed at anyone who's happy with what he's doing.
- She allows him to beat her up, physically or mentally, pressumably because of how she wants to view him.
- She beat up guys who were bullying him. IIRC she said something about protecting him, so maybe inverting the roles in her head. I don't really get it. It turned out to be a horrible idea either way.
- Natsuo beat Sanae probably only because Sawamura won against him easily in what I believe was a simple training match.
- Sawamura wasn't probably planning any real revenge until Natsuo went and utterly humiliated her karate club kouhais. Even without that they weren't on good terms for sure. You probably wouldn't be either if the person you considered a cute younger friend beat the living shit out of you for no reason (from your point of view at least).

Not only that, she lacks empathy. Like people who can't feel pain from birth are socially fucked up most of the time. I love how this manga explore the common shonen character trope (where eyebrow would have been a MC) and show how it is as fucked up as the rest.

I can't dislike eyebrows. Her enthusiasm is intoxicating.
